PLEASE PROVIDE THE FOLLOWING INFORMATION OR ITEMS WHEN SCHEDULING AN IME

1. Name and phone number (s) of claimant

2. Body part (s) to be evaluated

3. Individual and company requesting Independent Medical Exam and phone number

4. Attorney’s name and phone number, if any.

5. Copies of all medical records to be in our office 10 days before exam. If additional records are received after the scheduled examination, there will be an additional charge.

6. X-rays should be brought to Dr. Knoebel no later than the day of the examination by the scheduled examinee. If mailed, they need to be received prior to the examination day.

COVER LETTER INFORMATION

Once an appointment has been scheduled with our office, the following information is to be included in your cover letter.

1. Patient’s name and Social Security Number.

2. Confirmation of date and time of appointment

3. Claim Number or Case File Number

4. Date of injury

5. Employer

6. Claimant job title

7. Overview of case – reason for examination

8. List of specific issues and questions to be addressed in the report

9. Name of the company or individual to whom report is to be addressed

10. Need for an interpreter

11. Specific handling requests: Urgency of return/Time Sensitive, Privacy or Certified Mailing, etc.

12. Job site evaluation

*. Remember, with proper information from you, we can be more responsive to the issues outlined

* IME reports will be mailed to you in approximately 10 days.
